I don't think they are anti-patterns (and have not labelled them as such) - they are simply how teams work (e.g. the start-stop times).

If you have worked in a context where all those "anti-patterns" are absent then lucky you! For myself I (and every practitioner I know) have not had that joy. To be clear, I believe it is impossible to do it proper.

May be it's my misunderstanding over just a comment alone but it's also a bit of a weird argument. You could pick anything and say "yeah but when it's done proper what's wrong with it?"
